• Short term volunteer form
• Form A - The Medical Act 
• Work Permit Exemption Application Form 
• Certified Copy of Degree Certificate (Doctor of Medicine)
• Certified Copy of Current Licence
• Names and addresses of two (2) medical references 
• Two (2) photographs

* If you were trained at an Offshore Medical School and have a Board Certificate you will need to submit this.
